Inspire Therapy – Upper Airway Stimulation (UAS)

Inspire is the only FDA approved obstructive sleep apnea treatment that works inside your body to treat the root cause of sleep apnea. Inspire therapy is an implantable treatment option for people with obstructive sleep apnea who are unable to use or get consistent benefit from CPAP.

Dr. Karen is a fellowship trained in facial plastic and reconstructive surgery and has a separate facial cosmetic surgery practice located within his facility. He is board certified in facial plastic and reconstructive surgery and performs reconstructive surgery on the face related to skin cancer, trauma, congenital and other deformities.

Dr. Karen treats and performs procedures to address traumatic injuries to the face including laceration and fracture repair. He also removes facial skin cancers and repairs the resultant deformities with utmost care to make one look as normal as possible. Dr. Karen also repairs deformities that have resulted from facial paralysis where one side of the face droops because the nerves do not work.

Some of the products and brands that we offer include:

Behind-The-Ear (BTE) – This is the most traditional model: a small, curved case that fits behind the ear with a thin, transparent tube that runs into the ear canal. Newer models are far less visible than previous generation hearing aids. The BTE hearing aid is highly versatile, providing excellent treatment for most forms of hearing loss.

Open-fit Behind-The-Ear (BTE) – This is a smaller BTE with an “open-fit” that sits inside the ear canal yet prevents the ears from feeling occluded or plugged-up. This option is very comfortable and a great choice for people with mild to moderate hearing loss who are sensitive to having things in their ears.

Receiver-In-The-Canal (RIC) – This is a discreet, comfortable design that is easy to fit. It sits inside the ear canal similar to the BTE. A great choice for those with mild to moderate hearing loss who are sensitive to having things in their ears.

Completely-In-The-Canal (CIC) – Molded to fit deep within the ear, CICs are nearly invisible and tend to pick up less external (e.g. wind) noise because they’re protected by the ear itself. See a Miracle-Ear consultant to determine if you’re a candidate for this style. Typically appropriate for mild to moderate hearing loss.

In-The-Canal (ITC) — Smaller than an ITE and very discreet, the ITC fits partly into the ear canal. Due to its small size, the ITC comes with the option of remote control accessories for easy adjustment. Best for people with mild to moderate hearing loss.

In-The-Ear (ITE) – This small device is worn inside the outer ear and designed to match the wearer’s skin tone. It is discreet, yet offers many features and options that smaller hearing aids can’t. Best for people with mild to moderately severe hearing loss.

Primary Snoring is upper airway noise while sleeping. This is usually noticed by the partner during the course of the night. Snoring could be related to nasal obstruction or more serious sleep apnea problems. If deemed that snoring is just upper airway sounds and nothing more, Dr. Karen can suggest medical treatments, ie. weight loss, dental appliances to improve this night time sound, however, if these fail to correct the problem Dr. Karen can perform simple surgical procedures in his office to improve a patient’s snoring problem. These procedures are not covered by insurance but can be paid for as fee for service prior to the procedure. This will be discussed fully at the time of consultation.

More serious obstructive sleep apnea involves collapse of the upper airway during sleep with lack of oxygenation of body’s tissues. Obstructions can be in the nasal cavity oral cavity and deeper in the throat. The heart has to work harder as does the lungs, resulting in long term serious consequences. It manifests as day time tiredness and restless night time sleep. It is diagnosed with an overnight sleep study. The standard medical treatment is the use of a CPAP machine to push positive pressurized air down to the lungs to oxygenate the body. Many people unfortunately do not tolerate the daily use of the machine. Dr. Karen performs surgical procedures depending on the level of obstruction to improve a patient’s ability to breathe at sleep time. The most common obstruction is in the oral cavity due to enlarged tonsils, and redundant throat tissue. A UPPP (uvulopalatopharyngoplasty) is the standard operation that Dr. Karen performs.

Head and Neck Cancer

Dr. Karen evaluates neck abnormalities including cysts and masses. He does not evaluate thyroid disorders. He performs surgery on the neck routinely.

Dr. Karen is fully trained and evaluates and treats head and neck cancer. He performs diagnostic biopsies and surgical removal of various head and neck cancer problems. More involved cancer is referred to tertiary care facilities within our region.

Head and Neck Cancer refers to a group of cancers found in the mouth, throat and voice box region that can spread to the lymph nodes in the neck causing neck swellings. Risks factors for developing this cancer usually include smoking/chewing tobacco, and drinking alcohol in excess.

Together you and Dr. Karen will decide the correct path to take to treat your cancer. Sometimes this will include surgery and radiation, surgery alone, or a combination of surgery, radiation, and chemotherapy. If a referral is needed it will be discussed at your consult once a diagnosis has been made.

Dr. Karen treats all medical disorders of the ear. These include:Ears

  • Cosmetic repair (otoplasty)
  • Dizzy and balance disorders
  • Draining ears
  • Earaches (associated with TMJ)
  • Hearing loss – conductive/sensory neural
  • Otitis externa (swimmer’s ear)
  • Otitis media (ear infection)

He performs tubes in the office as well as the operating room. More serious ear surgical problems are referred to a tertiary care facility within the metropolitan region.

A full audiologic evaluation center is located within the practice. Dr. Karen has a staff audiologist who performs evaluations on Thursdays. The audiologist discusses hearing abnormalities and our practice provides hearing aid services at reduced costs.

 

Mouth and Throat

Mouth and Throat

Dr. Karen treats all infectious and inflammatory disorders of the mouth and throat. He treats salivary, inflammatory, infectious or neoplastic disorders. Dr. Karen performs surgeries including:

  • Esophagoscopy
  • Laryngoscopy
  • Salivary gland surgeries
  • Tonsillectomy

He treats vocal disorders both medically and surgically and often sees vocal professionals. Dr. Karen sees and treats sleep apnea patients both medically and surgically.

Dr. Karen evaluates and treats problems with swallowing (dysphagia), hoarseness, reflux disease, and professional voice problems.

  • Esophagoscopy – endoscopic evaluation of the esophagus
  • Laryngoscopy – endoscopic evaluation and treatment of the voice box
  • Salivary gland surgeries – removal of the Submandibular gland because of infections/stones or growths, removal of the parotid gland
  • Sleep Apnea Surgery – UPPP
  • Tonsillectomy and adenoidectomy

Nose

Nose

Dr. Karen is an expert in nasal anatomy and physiology and performs surgical corrections for all breathing abnormalities. In addition, he has had extensive training in sinus disease and is an expert in medical and surgical management of sinusitis. He treats nasal allergies and other forms of nasal inflammation. He treats nosebleeds (epistaxis), nasal infections and smell problems.

Procedures performed include:
